Requests for information by members of the public
How to request information
If you wish to request information about yourself or on behalf of someone else, please contact the Subject Access Request team.
All requests for information can be made to:

Please note that we locate 999 incidents by date, time and full/exact location. We cannot search by patient name or details.
Your request must include:
- Your full name and address
- The patient’s name
- The incident date, time and location
- What information you would like.
If you are the patient, we will need proof of your identity e.g. copy of your passport and utility bill.
If you are requesting information regarding someone else, we will need the patient’s signed consent and your proof of identity.
